CM23 - C-16-A Form (GUARDIANSHIP ACCOUNTING ORDER). This form is used in guardianship cases filed with the Court of Chancery, Register in Chancery, State of Delaware. It is completed by a guardian or co-guardians to report financial activities and expenditures related to the management of a protected person's estate, excluding the final accounting. The guardian affirms under penalty of perjury that the submitted accounting is accurate and truthful to the best of their knowledge. After submission, the Register in Chancery examines the accounting, verifies calculations and documentation, and certifies its correctness. The court then notifies beneficiaries that the accounting has been filed and available for review for thirty days.
