DSS-1816 - DISMISSAL OF ADOPTION. This form is used in the state of North Carolina in the General Court of Justice, District Court Division, before the Clerk when the court decides to deny the Petition for Adoption filed by the petitioning parent(s) seeking to adopt a child. The form includes information about the case, the reasons for denial, and the order to dismiss the adoption proceeding. It also indicates the custody of the child reverts back to the county Department of Social Services or licensed private child-placing agency, or to the person who had custody immediately prior to the filing of the adoption petition. Five DSS-1816 forms are filled in. After the Clerk of Superior Court signs and affixes seal, one form is retained in the Clerk of Superior Court's office; one form is given adopting parents; one form is given to the biological parent(s); one form is given to the director of social services or licensed private child-placing agency; and one form shall be forwarded within 10 days by the Clerk of Superior Court to the Division of Social Services, State Department of Health and Human Services. www.FormsWorkflow.com
