This form is used in North Carolina for adoptions when a parent, guardian, or guardian ad litem voluntarily consents to the adoption of a child by the adoptive parents. The form includes information about the child, the consent to adoption, and certification by a Notary Public or authorized official confirming the execution of the document. The original Consent to Adoption by Parent, Guardian Ad Litem of the Mother/Father, or Guardian of the child is to be sent by the Clerk of Superior Court to the Division of Social Services, State Department of Health and Human Services, attached to the Petition for Adoption.
