DSS-1803 - CONSENT OF CHILD FOR ADOPTION. This form is used in the State of North Carolina when a minor child voluntarily consents to their own adoption by the specified adoptive parents. The form includes information about the child, their consent to the adoption, and certification by a Notary Public or authorized official confirming the execution of the document. The child acknowledges that the consent is irrevocable except under specific circumstances outlined in the law. Form DSS-1803 is prepared in duplicate and is to be signed by the child being adopted who is twelve years of age or over when the Petition for Adoption was filed or who becomes twelve years of age before the granting of the Decree of Adoption. The original form is presented to the Clerk of Superior Court who then forwards it with the Petition and other Consents to the Division of Social Services, State Department of Health and Human Services.
