DSS-1806 - REVOCATION OF CONSENT TO ADOPTION BY PARENT, GUARDIAN, OR GUARDIAN AD LITEM OF THE MOTHER/FATHER. This form is used in the state of North Carolina to revoke consent to the adoption of a minor child by a parent, guardian, or guardian ad litem of the mother/father. The person revoking their consent declares that they are the legal parent, guardian, or guardian ad litem of the child and that they are revoking their previous consent to the adoption. The form must be acknowledged before a notary public or another authorized official. The person revoking this Consent to Adoption shall give written notice to the person specified in the Consent. Notice may be given by personal delivery, overnight delivery service or registered or certified mail, return receipt requested. The Revocation shall not be effective unless delivered to the appropriate person within the time period prescribed by law, except that notice by mail or overnight delivery service is complete upon deposit in mail, postage prepaid, or with service with delivery charges paid by sender, addressed to the person to whom consent was given at the address specified in the consent.
