O.C.G.A. § 31-32-4. GEORGIA ADVANCE DIRECTIVE FOR HEALTH CARE. This is a legal document that allows an adult to plan for future medical decisions if they become unable to communicate their wishes. Authorized under O.C.G.A. § 31-32-4, this form combines several prior documents into one, including a Durable Power of Attorney for Health Care, treatment preferences (often called a living will), and optional guardianship nomination. Through the directive, the declarant may appoint a health care agent, provide instructions about life-sustaining treatment, nutrition and hydration, pain management, and end-of-life care, and authorize decisions regarding organ donation and final disposition of the body. The form becomes effective only when the declarant is unable or chooses not to make health care decisions.
