AFFIDAVIT--SERVICE BY PUBLICATION—O.C.G.A. §9-11-4(e). This form is used in the Superior Court of the State of Georgia to support a request for service by publication when a defendant cannot be personally served. Filed pursuant to O.C.G.A. § 9-11-4(e), this affidavit requires the plaintiff or another knowledgeable affiant to swear under oath that the defendant either resides outside Georgia, has departed the state, or cannot be located within the state after due diligence. The affiant must state the defendant’s last known address and affirm that the current whereabouts are unknown.
