FORM DC-418 - AFFIDAVIT – DEFAULT JUDGMENT SERVICEMEMBERS CIVIL RELIEF ACT. This form is used in Virginia courts to comply with federal and state laws that protect active-duty military members in civil legal proceedings. Before a default judgment can be entered against a defendant or respondent who fails to appear in court, the plaintiff or petitioner must file this affidavit to state whether the other party is currently serving in the military.
