This is a required document under the New York State Workers’ Compensation Board (WCB) process for final settlements of workers’ compensation claims. This form accompanies the Section 32 Waiver Agreement (Form C-32) and confirms that the claimant understands they are waiving future workers’ compensation and medical benefits related to the case. It explains the rights to withdraw within 10 calendar days, the Board’s approval process, and conditions for payment of the settlement.
