C-312.5 - AGREED UPON FINDINGS AND AWARDS FOR PROPOSED CONCILIATION DECISION REPRESENTED CLAIMANTS ONLY. This form is used in Workers’ Compensation cases when the parties involved, including a represented claimant, the employer or carrier, and any other interested parties, have reached an agreement on the findings and awards to be included in a proposed conciliation decision.
