C-300.5 - STIPULATION. This form is used in New York to record an agreement between the parties involved in a workers’ compensation case regarding certain facts or proposed findings. By signing this stipulation, the claimant, employer or insurance carrier, and any other interested parties acknowledge that they understand the legal effect of their agreement and voluntarily consent to its terms. The form may also address attorney fee arrangements, substitution of legal representatives, and the withdrawal of pending appeals.
