CV-455 - PETITION FOR NAME CHANGE FOR MINOR CHILD UNDER 14. This form is used in Wisconsin circuit court when a parent, parents, or legal guardian seeks to legally change the name of a child younger than 14 years old. This form requires petitioners to provide identifying details, including their own names and addresses, the child’s current legal name, date and place of birth, and the name that appears on the child’s birth certificate. The petition must also identify the non-petitioning parent, if applicable, or establish the legal basis for bringing the petition, such as being the sole surviving or adoptive parent, a parent of a non-marital child with no established paternity, or the child’s legal guardian if both parents are deceased or their parental rights have been terminated.
