CV-428 - PETITION FOR TEMPORARY RESTRAINING ORDER AND/OR PETITION AND MOTION FOR INJUNCTION HEARING (INDIVIDUAL AT RISK - 30713). This is a Wisconsin Circuit Court form used to protect adults or elders at risk from abuse, neglect, or exploitation. The form allows the petitioner—who may be the individual at risk, a person acting on their behalf, or a representative of an adult-at-risk or elder-adult-at-risk agency—to request both a Temporary Restraining Order (TRO) and a court hearing for a longer-term Injunction Order. The form covers situations involving physical, emotional, or sexual abuse; financial exploitation; neglect; unreasonable confinement; or interference with protective services or investigations.
