CV-428 - PETITION FOR TEMPORARY RESTRAINING ORDER AND/OR PETITION AND MOTION FOR INJUNCTION HEARING (INDIVIDUAL AT RISK - 30713). This is a Wisconsin Circuit Court form used to protect adults or elders at risk from abuse, neglect, or exploitation. The form allows the petitioner—who may be the individual at risk, a person acting on their behalf, or a representative of an adult-at-risk or elder-adult-at-risk agency—to request both a Temporary Restraining Order (TRO) and a court hearing for a longer-term Injunction Order. The form covers situations involving physical, emotional, or sexual abuse; financial exploitation; neglect; unreasonable confinement; or interference with protective services or investigations. Petitioners can request specific relief, including restricting contact, prohibiting interference with care or investigations, requiring the surrender of firearms, or protecting household pets. If granted, an injunction may remain in effect for up to four years, and in severe cases involving homicide or sexual assault risk, for up to ten years or permanently.
