JP210 1-05 - BOND REQUEST. This form is used by an individual who has posted a bond in a criminal or traffic case to request how the bond funds should be handled. The bond poster certifies the amount posted and may request that the bond be converted to pay any fees and fines owed to the court in the case. The form allows the bond poster to indicate whether any remaining funds should be returned to them or released to another designated individual. If the bond is not explicitly released to someone else, it will legally be returned to the person who posted it, unless it has been forfeited. The bond poster must provide their name, address, and contact information, and they must swear or affirm that the information provided is true.
