CVRC8 - APPLICATION FOR ENTRY OF DEFAULT. This form from the Pima County Consolidated Justice Court is used by a plaintiff, counterclaimant, third-party plaintiff, or their attorney to request that the court enter a default against a defendant or respondent who has failed to file a timely response to a summons and complaint. The applicant must identify the party who was served and indicate the method of service, such as by process server, alternative service, publication, or certified mail (for small claims). The applicant must also declare whether the non-responding party is currently serving in the U.S. military, as this may affect the court's ability to enter a default judgment. Once the application is filed, the defendant is notified that they have ten court days (excluding weekends and holidays) to respond before default may be entered against them.
