This filing notifies the Maryland State Department of Assessments and Taxation (SDAT) that the foreign LLC no longer wishes to operate within the state. To complete the form, the LLC must provide its Maryland-registered name, the state or country where it was originally formed, and the signature of an authorized person confirming the cancellation. The filing fee is $0 for standard processing and $50 for expedited processing. Businesses can submit the form online via Maryland Business Express or file by mail or drop box, with payments made to SDAT.
