TRADE NAME AMENDMENT APPLICATION. This form is used to make changes to an existing trade name registration in Maryland. It allows the business owner(s) to update information such as the owner’s name, address, or the address where the trade name is used, or change the description of the business. This form is necessary when any of these details change after the original trade name registration. The filing fee for the application is $25, with an additional expedited service fee of $50 for faster processing. The form must be submitted to the Maryland Department of Assessments and Taxation (SDAT), along with the appropriate fee. If the owner is deceased, or there is a change in marital status or personal name, relevant supporting documents, such as a death certificate or marriage license, must be attached.
