Form 3DC33 - DECLARATION ON GARNISHEE TRANSFER; EXHIBIT(S) NOTICE TO EMPLOYER OF JUDGMENT DEBTOR; GARNISHEE INFORMATION. This form is used in the District Court of the Third Circuit, State of Hawai‘i, by a judgment creditor or their attorney to inform the court and enforce wage garnishment when a judgment debtor changes employment. This form declares that a valid court judgment remains unpaid and attaches a certified copy of that judgment as Exhibit A, along with a separate calculation of the current amount owed if it differs (Exhibit B). It states that although a garnishee order was previously granted against the debtor’s former employer, the debtor is now believed to be employed by a new employer. The form requests that garnishment be transferred to the new employer to continue the wage withholding process.
