It is required for a statutory trust formed outside of Maryland that wants to operate in Maryland. The form requires information such as the trust's name, date of formation, jurisdiction of formation, principal office address, nature of the business in Maryland, and the name and address of a resident agent in Maryland. If the trust has done business in Maryland prior to registration, an additional penalty fee must be included.
