PR-1819 - CLAIM AGAINST ESTATE (INFORMAL ADMINISTRATION AND FORMAL ADMINISTRATION). This form is used in Wisconsin probate proceedings—both informal and formal administration—to formally notify the court and the personal representative of a decedent’s estate that a creditor is asserting a financial claim against the estate. The claimant must provide their contact information, the nature and amount of the debt owed, whether the debt is currently due or expected to become due, any payments or offsets made, and how the claim should be classified under Wisconsin marital property law if the decedent had a surviving spouse.
