CONTROL EQUIPMENT SUMMARY ­ INSTRUCTIONS Microsoft Word version of CE-01 Adobe PDF version of CE-01 The purpose of CE-01 (state form 51904) is to summarize all of the equipment used to control air pollutant emissions. CE-01 is a required form for submitting information about control equipment to IDEM, OAQ. For the purposes of this form, the term "source" refers to the plant as a whole and NOT to the individual emissions units. Summary of Control Equipment This table is intended to summarize all of the equipment used to control air pollutant emissions. The identification numbers listed on this form should correspond to the emissions unit identified on the Plant Layout diagram and Process Flow diagram. 1. Control Equipment ID: Provide the identification number for each device used to control or limit air pollutant emissions. 2. Control Equipment Description: Briefly describe the control device. The description should be complete enough that an inspector can easily identify this equipment at your source. 3. Pollutant Controlled: List each regulated air pollutant for which emissions are reduced by the control device. If there are multiple pollutants controlled by the same device, list each pollutant controlled on a separated line. 4. Emission Unit ID: Provide the identification number for each emissions unit vented to this control device. If there are multiple emission units controlled by the same device, list each unit controlled on a separated line. 5. Stack / Vent ID: Provide the identification number for each stack or vent to which this control device exhausts. If there are multiple stacks or vents for the same device, list each stack or vent on a separated line. 6. Applicable Rule: If this equipment is required by a federal, state, or local regulation, provide the citation of the applicable requirements. If the equipment is not required by any applicable rule, write "NA". Examples of applicable requirements are NSPS (New Source Performance Standard), NESHAP (National Emission Standard for Hazardous Air Pollutants), SIP requirement, NSR/PSD permit condition, etc. In the space provided, you need to be specific and identify which NSPS or NESHAP Applies. For example, if the Plastic Parts Surface Coating MACT (Part 63 NESHAP) applies, you would write "40 CFR 63, Subpart PPPP". NOTE: If you are limiting emissions at the source in order to bank the reductions in the Air Emission Credit Registry, you must also submit form EC-01, Generation of Emission Credits. ***SUBMISSION OF CONFIDENTIAL RECORDS*** All information submitted to IDEM will be made available to the public unless it is submitted under a claim of confidentiality. Claim of confidentiality must be made at the time the information is submitted to IDEM, and must follow the requirements set out in 326 IAC 17.1-4-1. Failure to follow these requirements exactly will result in your confidential information becoming a public record, available for public inspection.
