Complete one form for each process unit (or group of identical process units). PI-12 is a required form for all air permit applications submitted to IDEM, OAQ. For the purposes of this form, the term "source" refers to the plant as a whole and NOT to the individual emissions units. If you do not provide the enough information to adequately describe the process, the application process may be stopped. Part A: Grain Elevator Summary Part A identifies the main parameters of the grain elevator operation. 1. Process Installation Date: Provide the date on which the process was installed. If the exact date is not known, provide the month and year of the installation. If the unit has not yet been installed, provide the anticipated installation date. If the unit has been modified or reconstructed since the original installation, include the most recent modification/reconstruction date. 2. Grain Variety: Identify each variety of grain processed in the grain elevator. If there is another grain applicable beyond the listed items, specify the grain under the "other" option. 3. Maximum Processing Rate: For each variety of grain processed. provide the maximum processing rate in units of bushels per year (bushel/yr). 4. Is the Grain Cleaned prior to processing? Specify whether the grain is cleaned prior to processing and includes the percent cleaned if applicable. 5. Is the Receiving Area open or closed? Specify whether the receiving area for the grain is open to the atmosphere or closed. 6. Loading Source: Identify all loading sources and indicate the percentage of the total loading time for each. If there is a loading source other than truck, rail, or barge, specify the loading source under the "other" option. Part B: Storage Details Part B details the parameters specific to the storage operations of the grain elevator. If there are multiple units that are identical in nature, capacity, and use, you may use one row to summarize the data for the identical units. Identical units must be of the same type, process capacity, and installation date. 7. Storage Units: Identify the type of storage units used. If there is another type of storage unit applicable beyond the listed items, specify the unit type under the "other" option. 8. Quantity: Specify the quantity of each type of storage unit used. 9. Unit IDs: Provide the emission unit identification numbers for each storage unit. The identification number listed on this form should correspond to the emissions unit identified on the Plant Layout and Process Flow diagrams. 10. Number of Times Filled Annually: Specify the number of times the storage units are filled annually. Storage Capacity: Provide the total storage capacity of each type of storage unit in bushels. Also provide the total storage capacity for the source. Part C: Grain Handling System Details Part C details the parameters specific to the grain handling system. If there are multiple units that are identical in nature, capacity, and use, you may use one row to summarize the data for the identical units. Identical units must be of the same type, process capacity, and installation date. 12. Grain Handling System: Identify the type of grain handling system used. If there is another type of grain handling system applicable beyond the listed items, specify the unit type under the "other" option. 13. Quantity: Specify the quantity of each type of grain handling system used. 14. Unit IDs: Provide the emission unit identification numbers for each grain handling system. The identification number listed on this form should correspond to the emissions unit identified on the Plant Layout and Process Flow diagrams. 15. Are the Conveyors Totally Enclosed? Specify whether the grain conveyors are totally enclosed or open to the atmosphere. 16. Are the Transfer Points Totally Enclosed? Specify whether the grain transfer points are totally enclosed or open to the atmosphere. 17. Spout Type: Identify the type of spout used for the grain handling system. If there is another type of spout applicable beyond the listed items, specify the spout type under the "other" option. Part D: Dryer Details Part D details the parameters specific to the drying operations of the grain elevator. If there are multiple units that are identical in nature, capacity, and use, you may use one row to summarize the data for the identical units. Identical units must be of the same type, process capacity, and installation date. 18. Dryer Type: Identify the type of grain dryers used. If there is another type of grain dryer applicable beyond the listed items, specify the type of grain dryer under the "other" option. 19. Quantity: Specify the quantity of each type of grain dryer used. 20. Unit IDs: Provide the emission unit identification numbers for each grain dryer. The identification number listed on this form should correspond to the emissions unit identified on the Plant Layout and Process Flow diagrams. 21. Dryer Specific Parameters: Provide the operating parameters for the type of dryer used. If a fuel other than natural gas is used, include the PI02F form identifying all fuels used for the process, and complete the applicable PI-02 forms for the combustion process. Part E: Emission Factors Part E identifies the emission factors used to calculate air emissions from this process. 23.
