WC-108a - ATTORNEY FEE APPROVAL.This is a Georgia workers’ compensation form filed with the Georgia State Board of Workers’ Compensation (SBWC) to request approval of attorney fees and, when applicable, litigation expenses. This form is used when an attorney seeks Board approval of a fee contract, an assessed attorney’s fee by consent of the parties, or resolution of a fee lien dispute. The form requires identification of the claim, disclosure of the fee structure (percentage or lump sum), and confirmation that any requested fee does not exceed the statutory 25% maximum under O.C.G.A. § 34-9-108 and Board Rule 108.
