5.904(e) - PHYSICIAN’S REPORT. This is a required probate court form used in Florida guardianship proceedings to provide the court with current medical information about an alleged incapacitated person or ward. Completed by a licensed physician pursuant to section 744.3675, Florida Statutes, this report documents the ward’s mental and physical condition, ability to live independently, and ongoing need for guardianship. The form also addresses whether the ward may be restored to capacity, in whole or in part, and identifies any specific rights that may be restored, such as the right to vote, travel, manage property, or make medical decisions.
