Description
RULE 5.903(a). LETTERS OF GUARDIANSHIP OF THE PERSON. This is an official probate court form used in Florida Circuit Courts to document and certify the appointment of a guardian over the person of an alleged incapacitated individual. Issued by the court after the guardian has taken the required oath and satisfied all statutory prerequisites, these letters formally authorize a plenary or limited guardian to act on behalf of the ward. The form identifies the guardian, the ward, the court, and the scope of authority granted, such as determining residence, consenting to medical or mental health treatment, and making decisions regarding the ward’s social environment. The letters also note statutory limitations and restrictions under Florida guardianship law. www.FormsWorkflow.com
