Form 002 - PROFESSIONAL GUARDIAN EMPLOYEE REGISTRATION FORM. This form is used in Florida to register employees of a professional guardian who have fiduciary responsibilities to wards. Required by the Office of Public and Professional Guardians (OPPG) under Rule 58M-2.001, Florida Administrative Code, this form must be submitted as part of the employer’s Professional Guardian Registration Form (DOEA/OPPG Form 001). The form collects identifying and contact information, counties of practice, education history, examination status, and continuing education compliance. It also requires submission or verification of criminal background checks (FBI and FDLE) and credit history for each employee.
