Description
DEBTOR’S ELECTRONIC NOTICING REQUEST. This form allows a debtor to enroll in, update, or deactivate electronic service of documents in a bankruptcy case . By filing this form, the debtor consents to receive notices and case documents by email instead of paper mail and waives the right to postal service for most filings. The form explains that emailed documents can be viewed once without charge and should be saved or printed. It does not apply to service of a summons and complaint or matters requiring service under Bankruptcy Rules 7004 or 9014. Joint debtors must file separate requests. www.FormsWorkflow.com
