NOTICE OF CORRECTION OF SOCIAL SECURITY NUMBER. This form is used to notify creditors and credit reporting agencies that an incorrect Social Security number was previously submitted in a bankruptcy case . The form identifies the debtor, lists the incorrect and correct 9-digit SSNs, and instructs recipients to update their records. Importantly, this notice is not filed with the court. Instead, the sender must mail it to all parties and credit bureaus, file an Amended Official Form B121 with the court, and separately file a Certificate of Mailing to document service.
