That the etitioner was charged with and adjudicateddelinquent of, asexoffense(s) incasnumbers: That the sex offense did not involve force and was only a crime due to the age of the victim; at the time ofthe commission of the sex offense, the victim was 13 years of age or older; and at the time of the commissionof the sex offense, the etitioner was than years older than the victim. 3. That the etitioner files contemporaneously with this petition the following supporting documentation: a. A certified copy of the adjudication requiring registration which includes adetailed description of thesexoffense; b. c. A list of each the etitioner is required toregister or has ever been required toregister as a sex offender d. A copy of the etitioner's criminal record; e.
