SJ-100 - AMENDMENT TO COMPLAINT. This is a San Joaquin County Superior Court form used to correct or update the name of a defendant in a civil complaint. The form allows a plaintiff to replace a fictitious (Doe) name with the defendant’s true name once it becomes known, or to correct an incorrectly stated name. When substituting a fictitious name, no court order is required; however, correcting an incorrect name requires judicial approval.
