SCLAC ADPT - DECLARATION OF MAILING OR OF INABILITY TO ASCERTAIN ADDRESS. This is a Los Angeles Superior Court form used in cases where service by publication has been ordered by the court. The form allows a declarant to confirm that a summons, citation, statement of damages, or other court document was mailed to a defendant, respondent, or citee after their address was discovered during the publication period. It also provides a method to declare that the party’s address could not be determined despite efforts made during the court-ordered publication period.
