LASC JUV 005 - REQUEST AND ORDER FOR TRANSCRIPT BY ENTITLED PARTY. This form is used in the Superior Court of California, County of Los Angeles, by individuals who are legally entitled to access juvenile court records under Welfare and Institutions Code section 827. This form allows a requesting party—such as an attorney, parent, or other authorized individual—to request the preparation of a transcript from a specific juvenile court hearing involving a minor. The requester must provide identifying details about themselves, the minor, the court hearing (including the courthouse, department, judicial officer, court reporter, and hearing date), and the reason for the request, such as for use in a future court appearance. The form also allows the requester to specify whether the transcript should be expedited and who should bear the cost of preparation—either the requesting party, the court, or another entity. Once completed, the request is reviewed by a judicial officer, who may grant or deny it. If granted, the order directs the court reporter to prepare the transcript in electronic or paper format.
