JD-FM-154 - AFFIDAVIT OF NONCOMPLIANCE WITH LICENSE SUSPENSION ORDER. This is a Connecticut Judicial Branch Superior Court form used in Family Support Magistrate matters to certify that an obligor has failed to comply with the conditions of a previously issued license suspension order.
