VN004 - AMENDMENT TO COMPLAINT. This form is used in the Superior Court of California, County of Ventura, to correct or update the name of a defendant in a civil case after the complaint has been filed. This form applies in two situations: when a defendant was originally identified by a fictitious name because the plaintiff did not know the true name at the time of filing, or when a defendant was listed under an incorrect name that needs correction. In the case of a fictitious name, the plaintiff may amend the complaint without a court order once the true identity is discovered.
