VN271 - NOTICE OF ASSIGNMENT OF MEDIATOR. This form is used by the Superior Court of California, County of Ventura, to notify the parties in a civil case that a court-appointed mediator has been assigned due to the plaintiff’s failure to timely file the required Stipulation to Mediator and Mediation Date form.
