Description

CCDR 0128 - ORDER FOR CIVIL ELECTRONIC MONITORING. This form from the Circuit Court of Cook County, Illinois, is used in domestic relations cases when a party, referred to as the Contemnor, has been found in indirect civil contempt of court. Rather than being held in jail, the court may order the individual to be placed on electronic monitoring through the Adult Probation Department. This allows the person to live at home while having their movements tracked, giving them the opportunity to work, attend school, seek employment, receive treatment, or participate in other court-approved activities necessary to purge their contempt. The form documents the contempt finding, the steps required to purge the contempt, and the conditions of electronic monitoring, including the contemnor’s address, personal information, permitted movement schedule, and compliance obligations. It also requires the contemnor to allow probation officers to check on them at any time and directs the Adult Probation Department to provide compliance reports to the court. Failure to follow the terms of the order or to purge the contempt may result in termination of monitoring and remand to the Cook County Jail. www.FormsWorkflow.com
