565 - AMENDED APPLICATION FOR C ERTIFICATE OF AUTHORITY TO CONDUCT AFFAIRS IN MICHIGAN. This form is used by foreign nonprofit corporations—organizations incorporated outside of Michigan—that are already authorized to conduct affairs in the state but need to update or correct the information previously filed with the Corporations, Securities & Commercial Licensing Bureau. This form allows a nonprofit to report changes such as a corporate name change, adoption or amendment of an assumed name, updates to the resident agent or registered office in Michigan, modifications to the organization’s principal office address, or changes in the type of affairs it will conduct in Michigan.
