DOING BUSINESS AS (DBA) REGISTRATION INFORMATION CHANGE FORM. This form is provided by the Utah Department of Commerce, Division of Corporations and Commercial Code to report updates to an existing DBA registration. With a $17 non-refundable filing fee, the form allows a business owner to change the business purpose, update the registered agent or address, modify the principal business address, or add or remove individuals associated with the business. Each change must be clearly listed, including full names, positions, and Utah street addresses (P.O. boxes may only be added after the street address). The form must be signed and dated by an authorized party under penalty of perjury. All submitted information becomes a public record under the Utah Government Records Access and Management Act (GRAMA §63-2-201), though business addresses may be used instead of personal addresses for confidentiality. www.FormsWorkflow.com
