It must be typed or computer-generated and submitted with a non-refundable $59 processing fee. The form collects essential information including the name of the corporation, its business purpose, and details about the corporation’s share structure, such as stock types and the number of authorized shares for each type. The applicant must also designate a registered agent—either an individual, business entity, or commercial registered agent—who is responsible for receiving legal notices on behalf of the corporation. The registered agent’s physical Utah address (not just a P.O. Box) is required. The form must list the name, address, and signature of at least one incorporator who is forming the corporation. A principal business address is also required, along with the names and addresses of officers and directors.
