UNIFIED JUDICIAL SYSTEM SCHEDULED COURT RECORDS DISPOSAL CERTIFICATION REQUEST. This form is used by court officials in Pennsylvania to request approval for the destruction or transfer of court records in accordance with Rule 507(b) of the Pennsylvania Rules of Judicial Administration. The form must be completed by the record custodian—typically a clerk or court administrator—and includes detailed information about the records proposed for disposal or transfer, such as the record title, inclusive dates, content description, retention schedule reference, and whether audit requirements have been met. It also documents whether the records have been converted to an archival format like microfilm or PDF/A, and specifies the storage medium and location of any security copies. The request is submitted to the District Records Retention Officer for review and certification that all procedural requirements have been met, particularly for destruction requests, which must confirm that the records have been preserved in an approved archival medium. The Administrative Office of Pennsylvania Courts (AOPC) then evaluates the request and determines whether to approve, amend, or deny it.
