ANNUAL GUARDIANSHIP PLAN. This form is a comprehensive report required under Florida Statute §744.367, which must be submitted annually by a guardian of the person. This form ensures the continued oversight and protection of individuals deemed incapacitated (referred to as “Wards”) by the courts. It is due within 90 days after the anniversary of the signing of the Letters of Guardianship and must include original signatures. The form provides the court with detailed updates on the Ward's living situation, medical condition, care arrangements, social activities, and any changes in their physical or mental abilities. Guardians must report all addresses where the Ward has resided, the frequency of their visits, involvement in care plans, and whether the Ward’s rights (such as the right to vote or marry) may be restored. Additionally, the guardian must disclose the Ward’s health insurance and public benefits, medications, therapy services, and any assistive devices used or needed. The plan also includes a required Annual Physician’s Report, which evaluates the Ward’s capacity to make decisions in key life areas. For minors, additional documentation related to school and development is necessary. If any of the Ward’s rights have changed, the guardian must petition the court to update them or provide a justification for why no change is needed. The guardian must declare any remuneration received for services on behalf of the Ward, affirm disaster preparedness steps, and certify that the report is accurate to the best of their knowledge. The report must be signed by the guardian, and, if applicable, by co-guardians, attorneys, and preparers.
