Description
INITIAL GUARDIANSHIP PLAN. This is a comprehensive document required under Florida Statute §744.362, which must be submitted within 60 days after a guardian is appointed by the court. It outlines the guardian’s plan for providing care and oversight for the ward (the person under guardianship) for a specified period. This plan ensures the court is informed of how the guardian intends to meet the ward’s physical, medical, mental health, personal care, social, and residential needs. It also includes detailed information about the ward's current living situation, preexisting medical directives (such as Do Not Resuscitate orders), current medications, assistive devices, disabilities, insurance coverage, and services or therapies the ward requires. The form requires original signatures from the guardian(s), preparer, and attorney, and it may also include a disaster preparedness plan. www.FormsWorkflow.com
