STATEMENT OF CLAIM. This form is used to file a claim against an estate in Florida probate court. It allows a creditor or other claimant to formally assert a debt or obligation owed by the decedent’s estate. The claimant provides detailed information about the basis of the claim, including the nature of the claim (whether it is due, contingent, or unliquidated), the amount owed, and whether the claim is secured by any property. The form also requires the claimant's personal and attorney contact information, along with a declaration under penalty of perjury stating the facts are accurate to the best of their knowledge. This form must be filed with the probate court to notify the personal representative and other interested parties of the claimant’s intention to seek payment from the estate.
