Form 5-7a - ORDER OF FILIATION (Non-Article 5/5-B). The New York Family Court form is an order that adjudges and declares paternity under FCA § 564 when both parents are before the court and the father waives filing a petition (FCA § 523) and a hearing (FCA § 531). The order may also, by consent, change the child’s legal name, and includes space for any additional directives. Standard notices explain how to vacate a default, and set appeal/objection deadlines: 30 days from receipt or service (or 35 days from clerk mailing) for judge-entered orders, and 30/35 days to file written objections to a support-magistrate order.
