Form 6-1-b - PETITION BY PERSON ON BEHALF OF PERSON UNDER 21 FOR APPOINTMENT OF A PERSON OR PERMANENT GUARDIAN. This New York Family Court form is used to petition for appointment of a guardian of the person—or a permanent guardian—for a youth under 21. The petitioner identifies the minor/young adult, proposed guardian, residence, and relationships; addresses best-interest factors; and discloses any ICWA status, parental status (including TPR, surrender, or death), and relevant safety or court-history information for the proposed guardian and adult household members. Permanent guardianship requires additional grounds and attachments (e.g., TPR/surrender orders or death certificates, and any required assessment).
