Form TPR-1 - PETITION (PERMANENT NEGLECT). This form is used to seek termination of parental rights for permanent neglect. It may be filed by an authorized agency, foster parent(s), the child’s attorney, or a guardian ad litem at the court’s direction. The petition alleges that, despite an agency’s diligent efforts, a parent failed for at least one year or 15 of the most recent 22 months to maintain contact with or plan for the child, and asks the court to commit guardianship and custody to an agency or foster parent(s).
