This form is completed by a guardian-conservator to report a minor’s death and request court termination of both the guardianship and conservatorship. The sworn petition states the original appointment date and the minor’s date/time of death, cites termination under SDCL 29A-5-505, and requires a certified death certificate (SDCL 29A-1-107(2)).
