Guardians Report {UJS-142}

UJS-142 - GUARDIAN’S REPORT. This form is used by court-appointed guardians to provide an annual (or initial) update to the Circuit Court on a minor or protected person’s condition, living arrangements, services received, and the guardian’s contacts and activities. The form also asks whether the guardianship should continue, be modified, or terminated, and includes optional requests for guardian compensation or expense reimbursement. A notarized Affidavit of Mailing is included, requiring the guardian to mail copies to parties and note service, plus an Objection Notice advising recipients they have 60 days to object under SDCL 29A-5-403.
