Form10-29a - APPLICATION FOR ORDER FOR ACCESS TO CHILD(REN) AND/OR HOME-Before Petition Filed. This form is used by authorized child protective services (CPS) workers to request a Family Court order granting access to a child or home before a child protective petition is filed. The applicant must identify the child(ren) involved, explain the circumstances leading to the request, and specify whether they were denied access to the child or residence during an investigation of suspected abuse or neglect. The form allows the court to authorize immediate entry or child interviews, including after-hours applications by telephone when the court is closed.
