Form 10-27 - PETITION FOR EXTENSION OF SUPERVISION OF RESPONDENT AND/OR RELEASE OF CHILD TO RESPONDENT OR NON-RESPONDENT PARENT OR LEGAL CUSTODIAN OR GUARDIAN. This form is used in the New York Family Court to request an extension of an existing order of supervision or release in a child protective proceeding. Filed by a Commissioner of Social Services or an authorized child placement agency, the petition must show good cause for extending supervision of a respondent or continued release of the child to a respondent, non-respondent parent, or legal custodian.
